An exciting opportunity has arisen for suitably qualified and experienced lead nurse to join our Integrated Discharge Service (IDS). We are seeking a highly committed and motivated individual who will ensure that we are providing a patient-focused discharge service to patients at the National Hospital for Neurology and Neurosurgery (NHNN).

The Integrated Discharge team is led by the Senior Nurse with management and support from four Band 7 specialist Sisters covering the services at the NHNN, Homeless support, Admissions Avoidance in the Emergency department (7 days) and Alcohol and Drugs specialist service. The team also work in the Emergency Department and Acute Medical Unit each day with patients and by working with our community colleagues to support the patients discharge and prevent readmissions.
This post is specifically responsible for the delivery of the discharge support service at the NHNN. The postholder will specialise in the care of people admitted with Neurological, Neurosurgical and stroke diagnosis and will demonstrate a high level of expertise in this specialty.

The team work closely with staff across all disciplines between hospital and community to ensure a safe, timely and effective transfer of patient care on discharge. They are also responsible for monitoring delayed discharges in line with the Community Care Act 2014, and developing systems to reduce the number of delayed discharges.

The post holder will also lead a team of nurses in continuing to build a discharge service that works across the Trust to ensure safe, timely and effective transfer of patient care on discharge. They will have the ability to build relationships with staff across all disciplines between both the hospital and within the community. They will also provide a strategic overview of the service and work to develop effective systems to reduce the number of delayed discharges at the NHNN. They will also deputise for the IDS Senior Nurse when requested.

We are looking for a dynamic and motivated individual with previous experience of managing a Discharge service, excellent communication skills and an innovative and flexible approach.
